How Neuromodulators Work to Smooth Out Your Wrinkles

Board-certified plastic surgeon Dr Jennifer Walden explains how neuromodulators like Botox, Dysport and Xeomin work to smooth out your wrinkles and crowds.

"They work by blocking the nerve signal. The small muscle injected into it is paralyzed, you can't move it, and so you get a smooth forehead and a crowfeet."

Dr. Walden recommends finding someone who is highly experienced in injecting Botox, Dysport or Xeomin, or fillers.
"Generally, I like to start with very light people because if you inject a lot of Botox for the first time they will feel treated and will not move their foreheads. This will scare patients and they will not do it again," said Dr. Walden.

There are new millennial trends that have gained Botox in the early 20s and early 30s. "If a patient comes to me and they don't need it I say wait. Wait until you have a reason to need it, until you are older and have children and worry and then we will treat you. I don't think it's in their interest to get them injected something into their body and pay for it if they don't need it, "said Dr. Walden.

Treating Facial Wrinkles that Appear at Rest, and Those that Appear in Motion Separately

One woman said she had lines under her eyes that formed when she smiled and relaxed. He wanted to know if he would benefit from Botox and fillers.

Dr. Amiya Prasad is a board certified cosmetic surgeon. He is a trained oculofacial plastic surgeon which means that his background training is first in eye surgery before he specializes in cosmetic and facial and facial reconstructive surgery. Eye aesthetics has been very important in practice in Manhattan and Long Island for the past 20 years.

Dr. Prasad explains that the lines that form when a person smiles are due to muscle activity, called dynamic lines. There is also a line that appears at a break called a static line. In addition, there are other lines that appear while smiling and skin rising. While smiling, he squeezed other lines that triggered rippling on the skin, which Dr. Prasad referred to as the effect of accordion.

When Dr Prasad discusses options with his patients, he thinks of short-term and long-term solutions. Skin changes are caused by many factors such as: genetics, heredity, environmental factors such as sunlight and smoking, as well as stress and hydration. The skin around the eyelids is about half a millimeter thick, so it's very thin and practically transparent. Trying to build collagen around this area and trying to improve this look should be done in a very good way.

The gold standard for dynamic lines is to use neurotoxin types such as Botox® or Dysport to reduce the muscle activity that causes these lines. The bottom line associated with the vertical lifting of the cheek cannot be filled with fillers. Such fillers in the hyaluronic acid family are too thick and make this area look swollen.

In the practice of Dr. Prasad, because the static line is essentially a function of collagen loss, he is trying to stimulate new collagen. The traditional method has always been about different types of heat energy devices and he is a big user of fractional CO2 lasers and Pellevé radiofrequency devices. He is not against thermal devices, but opposes too much. Many people have come to Dr. Prasad after they have been heated to the point where the skin becomes thinner and crêpey.

Dr. Prasad has been very successful in using platelet-rich plasma (PRP) derived from the patient's own blood plasma. It has the healing and growth factor needed for wound healing. Through strategic placement over time, PRP can build collagen on the skin. Dr. Prasad customizes this treatment based on the individual patient's response. There are some people who respond better than others.


People have wrinkles under the eyes and skin often, they have swollen bags or under holes. Dr. Prasad often combines PRP with a hyaluronic acid filler for the lower eye area. This combination is actually very effective in undergoing eye surgery and is something that her patients appreciate.

To address the issue of cheek volume, Prasad increases the volume in the areas of the constant, submalar and cheek. It is not to exaggerate but to restore the volume and continuity of the area with the lower eye area. People in their 20s and under the sun all the time have a lot of wrinkles under their eyes, but they still look good because they have a lot.

Dr. Prasad suggests that he finds the right doctor he is comfortable with and will guide him through this very difficult landscape. People who jump from one place to another spend a lot of money and don't get good results. Botox®, Juvederm and others are tools for skilled doctors and not their own solutions, so they are not a commodity. Rejuvenation for Under Eye Wrinkles, Eye Bags, and Hollowing Need Separate Treatments

A 42-year-old woman said she had terrible eyes and severe cramps. He wants to know the best procedure for solving the problem.

Dr. Amiya Prasad is a cosmetic plastic surgeon who has practiced in Manhattan and Long Island for over 20 years. Periodicals or aesthetics around the eyes are typical. This woman's situation is something she deals with everyday.

Hollowing is caused by genetics and aging. As people get older, they lose amounts of bone, muscle, and fat. At the bottom of the eye, especially at the junction of the eyelids to the cheeks, there is a condition called V-defect caused by the relative throat in the area. Cheeks also lose volume.
Every time Dr. Prasad saw the eyes of the people, he always saw their face. She does facelifts, cheek implants and many things to help rejuvenate her face. He thinks of everything separately, but also their relationship to other regions as the whole is always greater than the sum of its parts.

There are two types of facial wrinkles: wrinkles when people make facial expressions called dynamic wrinkles are activated during muscle contraction; wrinkles at rest are called static wrinkles. There are two different approaches to these issues.

Dr. Prasad also noted that while the woman's face was resting, there was swelling under her eyes, called lower eyelid fat prolapse. He usually performs a surgical procedure for this called transconjunctival blepharoplasty in which the fat causes the bag to be carved and reduced from the inside of the eyelids. This is done in his office under local anesthesia with LITE ™ sedation which means the patient has not had general anesthesia, so recovery is quicker. She explains that eye bags are the dominant female problem.

In order to lose volume below the eye, especially at the intersection of the eyelids and cheeks, Dr. Prasad discusses it with fillers such as Restylane, Radiesse and Juvederm, or uses more permanent solutions such as cheek implants. Most of the time, people don't want to hear about cheek implants the first time they see it. Occasionally, it is possible to have an injection test, and there is a value to the convenience of the injection compared to the surgery. However, injections are not permanent which means they need to be maintained.

As far as wrinkles are concerned, for static or wrinkles to rest, Dr. Prasad uses a combination of modalities consisting of platelet-rich plasma (PRP) and fractional CO2 laser. Static lines represent collagen loss. PRP, derived from your own blood, has a growth factor that stimulates collagen. He also uses lasers like fractured CO2 lasers. It is not intended to make a person look fine or porcelain, but to enhance the appearance of lines and wrinkles. When it comes to dynamic wrinkles, Dr Prasad uses Botox ™, Dysport, and Xeomin. This is an injection that can reduce muscle activity in the forehead or wrinkles. One thing to keep in mind is that when people smile, their cheeks go up vertically, so there will always be skin bunches in the area.

Dr. Prasad explained that it is important to understand how each of these factors contributes to the overall appearance. As a cosmetic surgeon, patients often come to her with high special attention on their priority list. He often lets them see themselves from a wider perspective. They learn that it may be important to them in terms of their perception, but they should consider how it contributes to their overall appearance. He can improve their appearance by taking bits and pieces of various issues and incorporating them. The results can be very good. He cannot stop the aging process, but he can certainly help patients see the best by addressing many of these genetic and aging problems.

Dr. Prasad suggests that it's best to meet an experienced, experienced cosmetic surgeon. How to Treat Under Eye Wrinkles and Dark Circles in Young People

First, he will use his own body healing factor to improve the texture of the skin with a leading edge procedure known as platelet rich plasma. This involves drawing a patient's blood (such as a blood test) and spinning until the platelets are concentrated. Platelets are the part of the blood that is responsible for freezing and healing. When carefully injected into the lower eyelid, it produces building collagen growth factor. It also stimulates new blood supply. Collagen is the backbone of the skin, and platelet-rich plasma is the only injection that can stimulate new blood supply, so the procedure has proven to improve skin quality and texture to address fine lines and dark circles. In addition, Dr. Prasad can also use laser and radio frequency devices to stimulate new collagen by using heat or radio frequency. This application causes collagen to tighten.

Dr. Prasad also recommends the use of injections to deal with hollowing and wrinkles below the eyes. The use of fillers such as Restylane can fill the interior of the eye tray. Dr. Prasad also saw a fine line in the photo where the woman was smiling or thinning. The subtle wrinkles that appear when smiling are known as dynamic lines. Even young women, if she has a genetic tendency toward fine lines, then Dr. Prasad would recommend considering regular injections such as Botox or Dysport to prevent premature lines.

In order to promote healthier skin under the eyes, Dr. Prasad also recommends a use of Eye Rescue from her skin care line. It works on the upper layers of the skin, and improves overall hydration and appearance. While this is not a treatment alone, it works well when combined with other recommended treatments.
