The Science of Facial Aging

Common changes that occur with facial aging include:
LOSS OF FAT
For many years we thought that human faces were uniformly aged. Now we know from Dr. Pessa that fats disappear from different fats in the face at different rates. Most cosmetic doctors now agree that fat loss rates are directly related to how old they are.
20-30 years Fat starts to disappear from under the eyes. Dark shadows make us look older and tired. Correction: Hyaluronic acid to lower eye area
30-40 years The descent of the Malar fat pad begins. The Nasolabial line appears. The result is wrinkles and pinks in the face. Those caused by muscle movement are referred to as dynamic wrinkles while those caused by skin aging are referred to as wrinkles. Correction: Hyaluronic acid to nasolabial line to hold constant fat pad. lower eye area
40-50 years The cheeks begin to level. The descent of the Malar fat pad becomes clearer. The Nasolabial Line deepens. Facial fat or waste atrophy becomes evident with contouring surface contours in the temple area and emerging cheeks. In some individuals, the eyes become submerged as a result of fat atrophy rather than forming an eyelid bag. Correction: Hyaluronic acids such as Sub-Q, Evolence or Radiesse to the middle face to hold a constant fat pad in place and achieve voluminisation. Fat deprivation is like autologous fat transfer. Marionette lines and jowls now appear. Double chin appears. Vaser LipoSelection, SmartLipo or Lipodissolve correction
50-60 years of menopausal effect. Fat hangs in the sagging skin. The nasolabial and marionette lines are deeper if not corrected. Marionette lines and jowls became clearer. Double chin and 'turkey neck' appeared. Correction Short-term wound repair. MACS facelift. Back neck lift. The advantages of fat appear under the eyes of Blepharoplasty Correction
LOSS OF COLLAGEN
Loss of collagen and elastin from the skin. Skin elasticity is reduced and skin is thinner. Lifestyle choices such as dietary exposure and sunlight affect the rate of these factors. Areas with thin epidermis (eyes and lips) first receive wrinkles.
20-30 years old Collagen collapses causing crows' feet and sweat lines. Correction: Botox narrows the line and around the eye area. Retinol or Vit-C
30-40 years of collagen and elastin levels continue to drop, causing the tip to melt. Extension of nasolabial folds. The lips start to thin. Glabellar (between eyebrows) and forehead wrinkles appear. With advanced filling techniques, we tend to see changes in the middle and lower faces. Correction: Botox to most areas of the face and to enhance eyebrows. Hyaluronic acids such as Restylane, Juvederm or Teosyl to enhance the lip and nasolabial line to maintain a constant fat pad. Sculptra and laser coating
40-50 years of collagen and elastin levels continue to decline causing bags and eyelids to appear on the upper and lower lip. Wrinkles wrinkles deep. Muscle and muscle pull causes the skin and structure to degrade or deepen from the deeper areas of attachment. Correction: Hyaluronic acid filler to upper lip. Botox to the top. Radiofrequency skin tightening treatments such as Polaris, ReFirme and Thermage. Infrared skin tightening like Titian and LuxIR. Removes skin lasers like ActiveFx.
50-60 years of menopausal effect. Fat hangs in the sagging skin. The nasolabial and marionette lines are deeper if not corrected. Neck wrinkles. More eyebrows. The tip of the nose is creeping. The lips are thin so there is less dry vermilion (pink area where lipstick is applied) shows. Perioral wrinkles deepen. Platysmal appeals appear on the neck. Correction: Hyaluronic acid filler to upper lip. Botox decreased. Face lift, Lift lift, Blepharoplasty or Neck lift are often required.
60-70 years old. Skin pigment cells increase in size and size in a cheating pattern that causes brown spots on the back of the hands and face (lentigo lush). Correction: IPL laser. Skin cancer screening
LOSS OF THE BON
The facial bones lose their density and thickness. This causes some minor changes in the shape of the face. Loss of bone along the spinal cord below the eyes and in the nasal area. The upper and lower jaw bones remain unchanged as long as the teeth remain intact.
20-30 years The small bone effect seen in this age group. No correction needed.
30-40 years The base of the nose gets bigger. Some changes are seen in the chin area. Correction: Enhances facial contour with hyaluronic acid to convert nasolabial lines to marionette ratio.
40-50 years old Chin area continues to grow. The eye socket begins to expand. Correction: Many cosmetic doctors find the changes to be subtle to require correction.
50-60 years of menopausal effect. Bone density decreases as a combination of factors such as decreased growth hormone and estrogen levels. Correction: Many cosmetic doctors find the changes too subtle to require correction.
